By Booking you, the parent or guardian of the child agrees:

If hired costumes are not returned in good condition within 2 weeks of the performance a fee of $50 will be charged to your dance account holder. Do not wash costumes.
to release Pulse Performing Arts including instructors and assistants, from liability for any and all injuries which may occur while training, practicing, performing, or during any studio event or activity. You give permission for emergency medical transportation and treatment of your student(s) at your expense should the need arise.
Pulse is not responsible for the personal belongings of any person using our services.
to give permission for photography/videography be taken of the child during activites with Pulse Performing Arts included and not limited to classes and performances to be used now or in the future for the purpose of external communications, including advertising and marketing as well as posted on the service's Social Media account (s) including Facebook, Instagram and website.
